We believe getting healthcare for your child should be simple and stress-free. Our process is designed to be transparent and convenient, from booking your visit to getting the care you need without the usual hassle.

Step 01: Reach Out & Schedule

The first step is to connect with us. You can easily inquire to learn more or book an appointment directly through our website. We aim to make scheduling flexible and accessible, especially for those unexpected sick days.

Step 02: Your Personalized Visit

Upon arrival, you’ll have a meet-and-greet with your doctor. This is where we’ll address your concerns, whether it’s for an acute illness, a sports physical, or a well-child check-up. Our focus is on providing comprehensive care and answering all your questions in a relaxed, unhurried environment.

Step 03: Simple, All-Inclusive Care

To make your visit easier, we offer many in-house tests—such as those for strep or other viruses—right on-site, often at a fraction of the cost of big clinics. If additional lab work is needed, we can arrange full-scope testing through Quest Diagnostics, also at highly competitive prices. And if a common prescription is needed, we can often dispense it on-site, saving you a trip to the pharmacy. You’ll leave knowing the full, transparent cost, with no surprise bills later.

Our Pricing

Simple, transparent care—no hidden fees, no insurance hassles. Whether you’ve been bringing your kids here for years or just need a one-time visit, every service at ReVision Wellness is designed to meet families where they are. Here’s what we offer:

Acute Illness Concern

Up to 20 minutes – $75
For straightforward, single health concerns that are new and have been present for less than a week, such as cold symptoms, sore throat, mild rash, ear pain, allergies, or recent vomiting Option to add a sibling for $50 (with a similar concern).

More Complex Concern

Up to 40 minutes – $150

For issues that are more persistent, such as belly pain that’s been lasting for several weeks, asthma management, or concerns with multiple symptoms. These cases may require a deeper discussion and more comprehensive evaluation.

Mental Health/Behavioral Health Evaluation

Up to 60 minutes – $225

For a detailed evaluation of mental or behavioral health concerns such as anxiety, depression, ADHD, or behavioral issues that are affecting daily life. This is an in-depth conversation that may include assessments, planning for support, and medication management (if appropriate).

Follow-Up for Established Issue/Concern

Up to 20 minutes – $75

For quick follow-up visits related to ongoing or previous concerns. These appointments are shorter and focused on progress, monitoring, or addressing any new questions or issues that may have come up.

Well-Child Exam

Up to 40 minutes – $150

For a comprehensive well-child exam, including growth and developmental assessments, vaccines, or just a discussion about any general health concerns or questions. 

Parental and Lifestyle Coaching

Up to 40 or 60 minutes – $150 or $225

For parents who need support or coaching on issues such as sleep challenges, difficult behaviors, picky eating (not related to growth), sibling issues, or general parenting advice. These sessions can be tailored to your family’s specific needs, and may be done virtually for the parents.

Sports Physicals or Daycare Forms

Up to 20 minutes – $75

For completion of sports physicals or daycare forms, outside of a wellness exam (where they would be included).

Medical Ear Piercing

Includes both ears, a 20-minute appointment, and aftercare instructions. – $100

This service includes a detailed discussion about the process, the sterile piercing itself, and aftercare instructions to ensure proper healing. Do you take insurance?

Nope—and that’s the point. We’re a direct primary care practice, which means no insurance billing, no hidden fees, and no surprise bills. You pay directly for care, just like you would at the dentist or chiropractor.

Do You Care for Patients with Medicaid?

At this time, Dr. Falk is unable to provide care for patients who are enrolled in Medicaid/ForwardHealth—even if families are willing to pay out-of-pocket. This is due to state regulations: if Dr. Falk were to provide care for Medicaid patients outside the Medicaid system, she could lose her designation as a "billing and performing provider," which would prevent her from continuing hospital-based work and newborn care in the region.

This is not a choice made lightly. It’s a limitation of the current system, not a reflection of the value of your family or your child’s care needs. We strongly support reforms that make healthcare more accessible for all families, and we hope to be able to offer more inclusive care options in the future.

Policy on Medicaid/ForwardHealth Patients

ReVision Wellness is a private, fee-for-service pediatric clinic and cannot provide care for children enrolled in Medicaid or ForwardHealth, even on a self-pay basis. Per Wisconsin state regulations, providing out-of-pocket care to Medicaid-enrolled patients may jeopardize Dr. Falk’s ability to continue hospital-based work under her current provider designation.

Families with Medicaid are encouraged to contact their assigned health plan for in-network pediatric care options. We recognize the limitations this places on access and hope to see changes in the system that allow for more flexible and inclusive care in the future.

Can my child see Dr. Falk if we have Medicaid but want to pay out-of-pocket?

Unfortunately, no. Due to state-level rules in Wisconsin, Dr. Falk is not allowed to see Medicaid-enrolled patients at ReVision Wellness—even if families are willing to pay privately. Doing so could result in the loss of her current Medicaid designation, which would prevent her from continuing important hospital-based work, including caring for newborns.

We know this rule feels frustrating and unfair. It’s one of the many ways the current system limits patient choice and provider flexibility. We hope these policies will evolve in the future to allow more families access to the care they want.

Can I use my HSA or FSA?

Yes! Most families can use HSA or FSA funds to cover the cost of ReVision Wellness services. We recommend double-checking with your plan provider to ensure accuracy.

What ages do you see?

ReVision Wellness cares for children and teens from birth through age 18. In the future, we may expand to include young adults and family medicine.

Where do you see patients?

Currently, we’re seeing patients at our clinic in Marshfield

Can you prescribe medications?

Yes—and even better, Amy can dispense many common prescriptions on-site. No need to wait at the pharmacy with a sick kid in tow.

How do I book an appointment?

You can call, or complete our online form. Same or next-day appointments are often available, and we’ll always try to find a time that works for your family.

Communication Policy

At ReVision Wellness, we strive to provide accessible and timely communication. You can contact me directly through the SigmaMD portal, and I am happy to respond to messages during clinic hours while I’m physically located in Marshfield. Please note the following:

  1. Communication Hours: I will respond to messages and questions during clinic hours when I’m physically in the Marshfield location. If your message is sent outside of these hours, please understand that I may not be able to respond until the next available clinic day.
  2. Urgent Issues: If you have an urgent concern or something that requires immediate attention (e.g., symptoms that may require urgent care or emergency care), please go to your local urgent care or emergency department. Messages received outside of clinic hours or for urgent issues may not be addressed right away, and it's important that you seek the care you need promptly.
  3. Response Time: For non-urgent matters, I typically respond within 2-3 business days. If you do not receive a response within this time frame, please follow up to ensure your message was received.
  4. Telemedicine Limitations: Due to noncompete restrictions, I am unable to provide telemedicine services when I am outside the Marshfield location. Therefore, communication through the portal should be considered for non-urgent inquiries and general guidance. If you're seeking advice or follow-up on previous visits, I can respond and assist you during clinic hours in Marshfield.
